JourneyCare Home Health, formally Addus Home Health, was founded over 40 years ago.
A lot has changed since then but our core objective remains the same: providing great care for the elderly, disabled, and chronically ill, especially those who are at risk of hospitalization or institutionalization.
We started out as a home cleaning service and then began providing chore services to Chicagos at-risk elderly population through a contract with the city.
In 2016, Addus refined its strategy to focus on growth in the states in which we have a current presence while adding clinical care services to our offerings.
We now provide personal care to over 42,000 patients while providing home health and hospice services to over 3,600 patients.
We now have 210 offices in 22 states.

Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
Identifies functional speech defects and establishes a plan of care to improve patients function.
Administer diagnostic speech and language evaluations and some basic, informal audio logical assessment in accordance with the plan of care Recommends communication devices/aids as indicated.
Prepare a report of each clients diagnostic evaluation and make recommendations Advise, consult and teach appropriate procedures and supportive activities to clients, family members, and other agency staff Provide direct speech service to clients under a providers plan of care.
Records and reports to the provider the clients response to therapy Qualifications: Possess certification of clinical competence in speech from the American Speech Language Hearing Association Licensed as a speech therapist in the state in which practicing At least one (1) year of acute care setting or equivalent experience Possess knowledge of principles and practices of speech therapy Complies and maintains current CPR certification.
