Job Summary :
The Stroke Program Manager plays a pivotal role in the development, implementation, and evaluation of the Stroke Program, ensuring the provision of high-quality services for patients experiencing stroke across the continuum of care. This position requires collaboration with the Stroke Program Medical Director to ensure program success.
The Stroke Program Manager is an expert nurse who provides leadership for the organization in services and systems necessary for a collaborative approach to stroke care.
The Stroke Program Manager plans, implements, and evaluates clinical programs to achieve high quality, patient-focused outcomes that advance patient and family care and nursing practice in stroke. The Stroke Program Manager is responsible for the development, quality assessment, performance improvement, and inter/intra disciplinary communications within the program.
The Stroke Program Manager will work collaboratively with all hospital and clinic personnel on shared system and performance improvement projects, while partnering with Strategic leadership to identify opportunities for the expansion of the program.
Job Responsibilities include, but are not limited to :
Lead the development, implementation, and evaluation of clinical programs within the Stroke Program to achieve high-quality, patient-centered outcomes. Establish operational and strategic goals aligned with standards set forth by the American Heart Association (AHA), American Stroke Association, Joint Commission, and New York State Department of Health (NYSDOH) to maintain Joint Commission Primary Stroke Center Certification. Collaborate with Quality Outcomes, Project Management, Risk Management, Case Management, Clinical Pharmacy, Regulatory Compliance, Nursing Education and Research, and other clinical leaders to lead quality improvement projects supporting the Stroke Program. Coordinate the development, implementation, review, and revision of Stroke Program standards, policies, procedures, and protocols. Participate in Stroke Alert activations, providing guidance as needed and ensuring adherence to protocols. Serve as a liaison with Emergency Medical Services (EMS), providing feedback and guidance on pre-hospital stroke processes. Coordinate the Telestroke Program and ensure its effectiveness. Apply quality improvement methodology to identify and lead improvement projects, utilizing tools such as PDSA and DMAIC. Facilitate delivery of acute stroke continuing education programs. Provide stroke-related education to internal and external stakeholders, including hospital personnel, EMS providers, local and regional communities, and patients. Stay informed of current stroke research and apply findings to practice. Actively participate in internal, local, regional, and national advisory groups to advance Stroke Care. Chair relevant Stroke Program committees related to Quality Improvement and regulatory compliance. Ensure accurate data registry abstraction and entry, including Get with the Guidelines through AHA. Utilize verbal and written communication skills to establish effective relationships, facilitate multi-disciplinary teams, and produce reports. Participate in Quality Committees and Morbidity and Mortality Conferences to drive quality improvement initiatives. Qualifications Required:
Education: BSN Required, master's degree preferred Licensure: Current unrestricted NYS RN License Work Experience: Minimum 3 to 5 years of clinical experience (inpatient and outpatient) necessary, 3 years in Program Leadership desired. Specific Skills: Excellent organizational skills, attention to detail, excellent communication skills (verbal and written). Experience working with Quality, Data, Performance Improvement, Customer Relations programs highly desirable. Physical: Ability to stand, sit or ambulate for long periods. Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems. Ability to transport self to customer base. Preferred:
Experience in program management, with a proven track record of successfully leading and managing healthcare programs. Familiarity with New York State regulations and Joint Commission standards related to stroke care and program management. Demonstrated ability to pursue and achieve publication in poster presentations or abstract submissions related to Quality Improvement (QI) work completed within the stroke program.
