Job Summary
•The Unit Supervisor is responsible for the supervision of care management staff and the daily care management operations of the program. This includes direct supervision of staff, ensuring quality of care management services provided to clients, management of staff documentation requirements, and staff training. The supervisor will ensure the provision of adequate clinical support to clinically complex clients in crisis. The supervisor will report to the Clinical Director. Essential functions include:
•Direct supervision to care managers to assist with the development of effective clinical interventions and care plans
•Ensure staff is kept informed of Health Home policies and procedures as well as departmental priorities. Receive appropriate trainings in order to maintain clinical standards
•Monitor Health Home compliance and quality assurance
•Provide crisis intervention to clients in distress
•Complete enrollment and service date tracking reports monthly or as needed
•Review and approve documentation as assigned
•Consistently exhibit the values of team work, open book management, advocacy and customer service
•Participate in quality assurance and quality improvement activities
•Conduct staff meetings, trainings and supervision
•Extensive field work is required to achieve client outcomes and meet health home requirements
•Performs other duties as requested
•Facilitation of bi weekly Clinical Case Review with the Care Managers
What You Need
•Bachelor's Degree Required; Masters Degree Preferred in Social Work, Psychology or Human Services.
Skills and/or Experience Required:
•Two (2) years post graduate experience working with individuals or families who have a history of mental illness, substance use, homelessness or HIV/AIDS
•Previous supervisory experience
•Excellent problem-solving, decision-making and judgment skills
•Ability to work on an interdisciplinary team
•You must have excellent interpersonal and time management skills.
•Strong leadership ability
•Preferred:
•MSW/LMSW/LCSW
•2 years working with severe mental illness population
•1 year supervisory experience
