General Summary of PositionResponsible and accountable for the provision of quality palliative care to patients enrolled in the palliative telehealth program, including collaborating with the palliative care teams at each site and primary medical teams to facilitate care. Facilitates improving patient outcomes by assessing and triaging patient needs through telehealth platform and overseeing the care provided by palliative care teams. Manages operational aspects of technology platform, marketing, quality improvement, data management and analysis related to outcomes and performance. Serves as a representative for the telehealth program at outreach events and other venues for promotion.
Primary Duties and Responsibilities Champions changes and acts as a liaison when implementing new programs to improve system processes and the quality of patient care.Contributes to the achievement of established department goals and objectives and adheres to department policies, procedure, quality standards and safety standards. Complies with governmental and accreditation regulations.Counsels associates effectively and collaborates with appropriate manager/director to utilize the disciplinary process.Demonstrates effectiveness in making independent decisions when necessary and in providing/obtaining assistance and consultation when appropriate.Ensures appropriate staffing and management of technology resources for the telehealth program.Investigates and follows through on customer complaints and incidents and appropriately communicates to management.Leads team collaboration efforts to serve and comprehensively manage the full circle of care for the Telehealth patient population.Monitors program to assure an environment that is conducive to the physical, spiritual, and emotional well being of the patient and to the development of staff.Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.Provides input for evaluating palliative care clinicians at each site related to their role in the telehealth program.Provides input to budget preparation. Assists in monitoring budget compliance, justifying any variances.
Minimum Qualifications EducationBachelor's degree required andMaster's degree preferred
Experience10+ years Related clinical experience in clinical setting caring for older adults required
Licenses and Certifications Candidate must have either an active DC or Maryland license upon hire, and must apply for the other jurisdiction within 1 month of starting. required Current RN license in the State of Maryland preferred
