Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care. · Social Worker: Works collaboratively with RN case managers and the multidisciplinary team, performs assessments and develops appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs. Engages in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
