**Job Title:** Registered Nurse **Job Specialty:** Home-Hospice Care Operations **Job Duration:** 13 weeks **Shift:** 8-hour Day Shift (40 hours per week) **Guaranteed Hours:** Not specified **Experience:** At least 1 year of Home Health Case Management experience **License:** WI RN license or compact **Certifications:** BLS **Must-Have:** - Registered Nurse with experience in Home Health Case Management - BLS certification - Valid RN license in WI or compact license **Job Description:** - Responsible for case management of 20-25 patients - Completion of in-home visits for admission, routine visits, recertification, and discharge - Development of skilled nursing care plans - Conduct visits based on the care plan - Collaborate with interdisciplinary team and provider offices to ensure optimal patient outcomes - Provide care for patients with multiple comorbidities such as CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, falls, fractures, infections, cellulitis, post-surgical conditions, wound care, ostomy care, labs, and IV infusion - Any scrub color is acceptable - Must provide their own transportation for patient visits - Coverage area includes specified market regions and counties
