**Job Title:** Registered Nurse - Home-Hospice Care Operations **Job Specialty:** Home-Hospice Care **Job Duration:** 13 weeks **Shift:** 8-hour Day Shift (40 hours per week) **Guaranteed Hours:** Information not provided **Experience:** 2 years of hospice experience, travel experience, or EPIC Remote client experience preferred **License:** Compact or WI RN license **Certifications:** BLS (American Heart Association) **Must-Have:** - Comfortable providing end of life care and planning - Proficient in wound assessments and care coordination - Skilled in pain/symptom management - Ability to triage appropriately - Experience with EPIC Remote Client preferred **Job Description:** - Service area includes North Milwaukee and the counties of Washington, Ozaukee, Waukesha, and Jefferson. - Provide 1:1 patient care with support from other Case Managers, RNs, and a Resource Nurse. - Deliver skilled end-of-life care to adult patients. - Utilize Sapphire IV pumps for patient care. - Up to 1 week of orientation available based on comfort level. - Flexibility in attire: any color scrub or business casual is acceptable.
