**Job Title:** Registered Nurse **Job Specialty:** Home Health - Hospital at Home **Job Duration:** 13 Weeks **Shift:** Rotating 12-Hour Shifts; Day (07:00 - 19:00) and Day (11:00 - 23:00) **Guaranteed Hours:** N/A **Experience:** Minimum 3 years of critical care, acute care, or medical-surgical RN experience **License:** VA or compact license required at the time of submission **Certifications:** BSN required **Must-Have:** - EPIC charting experience preferred - Minimum 1 year of experience - Ability to work collaboratively with a multi-disciplinary healthcare team **Job Description:** - Provide direct patient care to inpatients at the acute level within the comfort of their home environment. - Develop and follow an individualized plan of care for each patient during home visits. - Utilize leveraged technology and remote clinical care coordination to deliver essential inpatient care. - Implement the nursing process through a team nursing model to ensure high-quality and safe patient care. - Access, document, and assess patient care, delivering therapeutic interventions. - Assist with rotating shifts as needed to provide comprehensive care. - Complete non-billable modules during orientation, averaging 6 hours.
