**Job Title:** Home Health and Hospice Registered Nurse (RN) **Job Specialty:** Home Health RN, Hospice RN **Job Duration:** 13 weeks **Shift:** Tuesday-Saturday 8am-4pm, on call required **Guaranteed Hours:** 37 hours per week **Experience:** Minimum 2 years of experience **License:** Registered Nurse License **Certifications:** Relevant nursing certifications **Must-Have:** Experience with OASIS documentation, proficiency in high-tech nursing skills (IV use, wound care) **Job Description:** - Utilize the nursing process to assess patient/caregiver needs to provide quality care to assigned patients, in accordance with evidence-based practice, State and Federal regulations, and nursing board policies. - Develop and communicate a realistic plan of care for each patient, ensuring involvement of patient/caregivers in the process. - Serve as Case Manager for assigned patients, managing case loads and visit times in adherence to agency productivity standards. - Complete OASIS documentation effectively, staying current with documentation requirements and ensuring timely completion. - Implement medical plan of care, nursing interventions, and procedures to ensure safe, effective patient care. - Maintain high-tech skills, including IV use, wound VAC, and complex wound care, while upholding infection control standards. - Collaborate with LPNs, delegating tasks to ensure the plan of care is followed and patient needs are met. - Evaluate implemented plans of care, assess aspects like identified problems, care provided, and patient responses. - Maintain accurate documentation of nursing care to facilitate effective information integration for patient care. - Follow agency protocol with processes, including visit completion and documentation. - Conduct patient and caregiver education to enable informed decisions regarding treatment options and other relevant information. - Identify and document interdisciplinary needs, communicating timely with relevant teams or services. - Fill out necessary agency, state, and federal forms accurately and in a timely manner. - Carry out visits efficiently, ensuring supervisory visits are not combined with routine visits as necessary. - Assess patient needs for services, developing and coordinating care plans with patients and caregivers. - Report patient incidents like infections or injuries promptly to the appropriate specialist. - Coordinate safe patient discharges or transfers, ensuring all paperwork and information are processed properly. - Coordinate a range of patient care activities with physicians, providers, team members, and vendors to support the recommended plan of care. - Participate in monthly peer review audits, providing and accepting constructive feedback professionally. - Schedule, coordinate, and facilitate case conferences to maximize effective team communication. - Advocate proactively for patient care issues to ensure care is sensitive to each patient/family's needs. - Conduct comprehensive assessments of patient/caregiver needs as part of the nursing process. - Participate actively in interdisciplinary team meetings to develop individual plans of care, with assessments conducted every fifteen days. - Stay updated on pain control, symptom management, and high-tech nursing skills. - Educate patients and caregivers to enable informed decisions regarding health care and treatment options.
