**Job Title:** Registered Nurse **Job Specialty:** Hospice **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** Not guaranteed **Experience:** Minimum 1 full year of Adult Home Health or Hospice experience **License:** Must have a valid nursing license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in HomeCare HomeBase electronic medical records (EMR) - Experience with OASIS preferred - Reliable transportation, current driver's license, and auto insurance - Willingness to travel up to 75 miles from the home clinic - Must complete all documentation within 24 hours of patient appointments **Job Description:** - Provide, coordinate, and direct the provision of home nursing care according to physician's orders and based on agency policies and procedures. - Apply nursing process competently to ensure quality care. - Responsible to the Clinical Manager and directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Manage an average of 5-8 patients per day, ensuring all necessary documentation is completed promptly. - Handle various cases including Start of Care, Wound Care, Follow-up Visits, etc. - Participate in on-call rotations as required every 2-3 weekends (back-up only). - Report to the designated location in Northwest Missouri for assignments and attend weekly meetings. - Travel approximately 50-100+ miles per week as part of the job responsibilities. - Fulfill weekly productivity expectations of 20-25 units. Note: This is a home health assignment requiring daily driving to patient's homes. The clinician must be aware of and comfortable with travel requirements.
