First Time travelers Accepted: Yes IN STATE LICENSE / REQUIRED CERTS MUST BE IN HAND TO SUB Minimum Experience Required: 2 Years Required Department Details: 5C is a 31 bed med-surg unit with a concentration in general surgery, urology, gynecology and bariatric surgery. Must have 3-4's in Bariatric Surgery, Thyroidectomy, Bowel Obstruction, and CBI Patient Ratio: 1:6 Effective 9/1, the COVID vaccine is required for all travelers at IU Health campuses. Declinations will not be accepted; however medical or religious exemptions will be accepted with a doctor's note provided. Job Requirements Certification Requirements: BLS Floating Requirements: Yes Additional Details Valid Driver's License or Passport accepted. Must be clear copy. Online Verification of License to include expiration date and any disciplinary actions (within 30 days before start date). Must be through primary source. IN.gov. Proof of valid auto insurance required for all travelers under Hospice specialty. Must be current at start, but expiration does not need to be tracked and the doc on file does not need to be renewed.
