**Job Description:**
- Utilize the nursing process to provide quality patient care according to established standards of care, including planning, implementing, evaluating, and adjusting plan of care based on expected patient outcomes.
- Admit, transfer, and discharge patients efficiently and accurately.
- Complete ongoing patient assessments and documentation according to policy.
- Develop patient plans of care including problems, goals, evaluations, and interventions.
- Report symptoms and significant changes in patient condition to appropriate personnel promptly.
- Assess, collaborate, and coordinate discharge planning needs and readiness for discharge with other members of the care team.
- Demonstrate the appropriate application of age-specific standards, policies, and procedures in caring for diverse patient populations.
- Incorporate cultural and developmental needs into the care plan.
- Demonstrate critical thinking skills and ability to delegate tasks to staff based on patient assessment.
- Provide comprehensive documentation of patient care in accordance with standards.
- Collaborate with physicians and the multidisciplinary healthcare team for patient/family education regarding disease processes, wellness promotion, and discharge planning.
- Participate in performance improvement activities, including data collection, analysis, and identifying measures to maintain quality and promote desired outcomes.
- Demonstrate awareness and relevance of research in nursing, evaluating practices to identify areas for improvement or research opportunities.
- Effectively communicate and utilize departmental tools and systems for information sharing with the healthcare team.
- Exhibit the hospital's philosophy, mission, and values through actions and communication, incorporating them into the working environment.
- Maintain confidentiality related to all aspects of patient care and adhere to the confidentiality policy.
- Promote a cooperative work environment with a positive attitude and effective communication.
**Physical Requirements and Environmental Exposures:**
- Ability to lift, carry, push, and/or pull up to 50 pounds while administering patient care.
- Engage in stooping, kneeling, crouching, standing, and walking for five or more hours per day.
- Visual acuity to distinguish skin tone variations, IV infiltrations, infections, and read telemetry and blood pressure gauges.
- Audio acuity to distinguish body sounds and detect equipment operation issues.
- Handle environmental hazards including chemicals, solvents, sharps, blood, bodily fluids, infectious diseases, and medical waste, while utilizing necessary protective clothing/equipment.
