- Minimum of 2 years of recent experience as a Wound Care Coordinator
- Valid RN license for North Carolina
**Job Description:**
- Manage the facility wound care program, including standards of care and practice for wound, ostomy, and continence patient care.
- Provide direct patient care, assistance to staff nurses, and consultative services to affiliated healthcare agencies.
- Develop and implement the facility wound care program in accordance with national standards, including patient care protocols, documentation tools, wound care formulary, and referral criteria.
- Establish and maintain standards of care, competencies, policies, and procedures for effective, quality, and cost-efficient wound care across all clinical staff.
- Act as the primary consultant to Wound Care Clinicians and serve as a consultant to facility staff. Advocate with physicians, providing training and support to enable effective wound assessments, protocol recommendations, and care plan initiation.
- Provide ongoing education to staff on available products for hospital use.
- Evaluate all wounds upon admission and ongoing to determine treatment plans and identify problems early.
- Provide consultation and assessment for patients with pressure injuries and wounds not showing measurable healing within two weeks.
- In consultation with physicians, assist primary nurses in developing appropriate plans of care for comprehensive wound management and prevention.
- Make recommendations for changes to wound care orders, supported by evidence-based research.
- Review medical records of patients with wounds to ensure proper documentation and treatment.
- Investigate adverse events related to wounds, complete root cause analysis, and develop action plans in conjunction with nursing leadership.
- Participate in clinical outcome monitoring, follow-ups, and performance improvement initiatives.
- Contribute to CMS quality data reporting through wound-related data set records.
- Collaborate on maintaining a cost-effective wound care formulary.
- Assist with training new hire Wound Care Coordinators as needed.
