**Experience:** Minimum of 2 years with strong orthopedic experience; familiarity with neuro and some acute cross-coverage preferred.
**License:** Licensed by the Examining Committee to practice Occupational Therapy in the state required. Compact License acceptable if worker is in a state that is part of the OT Compact agreement.
**Certifications:** Basic Life Support (BLS) required (American Heart Association or American Red Cross).
**Must-Have:** Valid state driver's license and auto insurance; personal vehicle must be well-maintained and safe for driving.
**Job Description:**
- Evaluate and manage an assigned group of patients, overseeing and providing occupational therapy services in the home setting.
- Provide direct patient care and supervise Certified Occupational Therapy Assistants (COTAs) according to established plans of care.
- Coordinate care with a multidisciplinary team and serve as the patient's liaison with other healthcare providers in the continuum of care.
- Ensure appropriate and timely patient care and documentation to meet patient needs and organizational goals.
- Design and implement therapy programs aimed at restoring function, preventing disability, and promoting independence in activities of daily living.
- Act as a case manager for post-acute services, including the supervision of home health aides.
- Develop plans of care in collaboration with patients' physicians and assess patients' functional abilities.
- Design and fit patients with orthotic devices and adaptive equipment; instruct on therapy methods.
- Monitor and report patients' reactions to treatments and any changes in condition to physicians.
- Communicate effectively with the multidisciplinary team for updates and adjustments in care plans.
- Delegate appropriate care elements to qualified healthcare workers under minimal supervision.
