**Job Description:**
- Ensure all credentialing requirements are met prior to submission.
- Attach three references with submission.
- Complete a county criminal check covering seven years for all counties lived and worked in.
- Conduct a federal criminal check covering seven years or more.
- Perform a multi-state criminal search for seven years or more.
- Complete a nationwide sex offender search for seven years or more.
- Conduct an HHS/OIG search.
- Perform a GSA search.
- Complete a Social Security Number trace.
- Provide 2 Step PPD or Quantiferon test results, or proof of positive PPD and Chest X-Ray.
- Conduct a SAM search.
- Ensure a physical examination is conducted.
