- Proficiency in HomeCare HomeBase (HCHB) EMR
- Experience using Oasis within the last 6 months or willingness to complete a retraining course
**Job Description:**
- Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Perform home visits and attend periodic office meetings.
- Travel required up to 75 miles from the home clinic with mileage reimbursed at the IRS standard rate.
- Manage an average of 5-8 patients per day, corresponding to roughly 6-8 points, with a weekly productivity expectation of 25/30.
- Handle various types of cases including Start of Care, Wound Care, and Follow-up Visits.
- Complete all documentation within 24 hours of patient appointments to ensure billing and follow-up patient care are not impacted.
- Open to floating to nearby clinics if patient census is low to meet financial needs.
- Must be comfortable with drives potentially longer than 20 minutes between patient locations.
- Participate in a home health assignment involving daily travel to patients' homes and weekly clinic meetings.
- Not required to be on call and have no weekend rotations, although this may vary per location.
