**Job Description:**

- Conduct comprehensive assessments to evaluate patient needs.
- Develop and implement personalized treatment plans.
- Assist patients in improving their daily living and work skills.
- Monitor and document patient progress and modify treatments as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Maintain a clean and safe working environment for patients.
- Follow required credentialing checklist for background checks and health screenings, including but not limited to:
- Social Security Number Trace
- Criminal Background Checks (County, Federal, Multi-State, Nationwide Sex Offender Search)
- Health and Human Services/Office of Inspector General Search
- General Services Administration Search
- System for Award Management Search
- Tuberculosis Testing or Proof of Positive Test and Chest X-Ray
- Provide compassionate and quality care to all patients.
