License: State License Verification required

Certifications: COVID-19 test required 72 hours prior to start date with results issued 48 hours prior

Must-Have: Prescreen filled out and attached, resume, and license verification

Job Description:
- Ensure the prescreen is filled out and attached when submitting the application
- Attach a resume and license verification; incomplete submissions will result in the candidate being declined
- Conduct a state license verification and provide three references
- Complete background checks including County Criminal (7 years for all counties lived and worked), Federal Criminal (7 years or more), Multi-State Criminal Search (7 years or more), and Nationwide Sex Offender Search (7 years or more)
- Perform HHS/OIG, GSA, and SAM searches
- Provide a Social Security Number Trace
- Submit proof of 2-step PPD or Quantiferon, or proof of positive PPD and chest X-ray
- Undergo Oregon State Background - CRIMS (facility run)
