**Job Description:**

- Provide occupational therapy services to clients in their homes.
- Develop and implement individualized treatment plans.
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
- Assess patients' functional abilities and provide interventions to improve activities of daily living.
- Educate patients and families on therapeutic techniques and adaptive equipment.
- Document patient progress and maintain accurate records.
- Travel to multiple locations within the assigned counties.
- Deliver compassionate and patient-focused care to enhance quality of life.
