**Job Description:**

- Provide occupational therapy services in a home health setting.
- Develop individualized treatment plans based on clients' needs and goals.
- Collaborate with multidisciplinary teams for comprehensive care.
- Conduct patient assessments and document progress accurately.
- Educate patients and their families on therapeutic exercises and strategies for independent living.
- Adjust therapeutic approaches based on patient progress and response.
- Participate in team meetings to discuss patient care and treatment options.
- Maintain up-to-date records in accordance with regulatory and company policies.
- Travel to patient homes and provide therapy services as per schedules.
- Adapt to varying home environments while ensuring a high standard of care.
