**Certifications:**
- BLS Certification
- National Accreditation through NBCOT

**Must-Have:** Master's Degree in Occupational Therapy

**Job Description:**
- Provide occupational therapy services to medically compromised patients, including those with CHF, COPD, DM, cardiac issues, orthopedic conditions, neurological conditions, dementia, and general weakness.
- Patient ages range from early adult to geriatrics.
- Conduct evaluations and determine effective intervention strategies.
- Perform medical assessments and develop comprehensive care plans.
- Collaborate with the Home Health Department team, which includes RNs, LPNs, PTAs, SLPs, aides, social workers, and case managers.
- Complete charting and documentation using the EPIC system.
- Participate in orientation, which includes shadowing and learning the documentation process.
- Floating may be required within the designated area.
- No weekend rotation or on-call duties; the office is closed on six major holidays.
- Scrub color is flexible; business casual attire is also acceptable.
