**License:** South Dakota Licensure

**Certifications:** BLS, National Accreditation through NBCOT

**Must-Have:** Master's Degree in Occupational Therapy

**Job Description:**

- Provide occupational therapy to medically compromised patients, including those with CHF, COPD, DM, cardiac conditions, ortho, neuro, dementia, and general weakness.
- Serve a diverse age group ranging from early adult to geriatrics.
- Perform evaluations, determine effective interventions, and conduct medical assessments.
- Develop and implement care plans tailored to individual patient needs.
- Collaborate with a multidisciplinary home health team, including RNs, LPNs, PTAs, SLPs, aides, MSWs, and case managers.
- Utilize EPIC for documentation.
- Participate in a one-week orientation process covering EPIC, shadowing, and documentation procedures.
- Adhere to a no weekend or holiday rotation schedule, with office closed on six major holidays.
- Wear any color of scrub or business casual attire, avoiding denim or sweats.
