- Completion and attachment of prescreen form

**Job Description:**
- Conduct comprehensive occupational therapy assessments
- Develop personalized treatment plans for patients
- Provide therapeutic interventions to enhance patients' abilities in daily activities
- Collaborate with a multidisciplinary team to ensure optimal patient care
- Maintain accurate documentation of patient progress and treatment outcomes
- Ensure compliance with state and federal regulations
- Fulfill 8 hours of dementia training
- Pass necessary background checks, including county and federal criminal searches and a nationwide sex offender search
- Conduct necessary health checks, such as PPD or Quantiferon tests or chest X-Ray for positive PPD cases
