**Job Description:**

- Provide comprehensive occupational therapy services to patients in a home health setting.
- Develop and implement individualized treatment plans to address patients' physical and functional needs.
- Collaborate with a multidisciplinary team to coordinate patient care and optimize outcomes.
- Conduct thorough assessments and re-evaluations to monitor patient progress and adjust treatment plans as necessary.
- Educate patients and family members on therapeutic exercises, safety, and adaptive equipment.
- Document all patient interactions, treatment plans, and progress notes accurately and timely.
- Ensure compliance with all regulatory and accreditation standards.
- Participate in team meetings and contribute to the evaluation and planning of services.
- Demonstrate strong communication and organizational skills.
- Maintain professional and ethical standards of practice.
