- 2 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray

**Must-Have:**
- Covid test required 72 hours prior to the start date with results issued 48 hours prior to start date
- Complete prescreen, resume, and license verification

**Job Description:**
- Ensure all required documentation is filled out and attached upon submitting your application

- Provide three references
- Complete background checks including:
- County Criminal (7 years for all counties lived and worked)
- Federal Criminal (7 years or more)
- Multi-State Criminal Search (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- SAM Search
- Oregon State Background - CRIMS (facility run)
- Provide quality care to patients in need of occupational therapy
- Collaborate with other members of the healthcare team to develop comprehensive care plans
