**Job Description:**

- Provide therapeutic interventions to help patients develop, recover, and improve the skills needed for daily living and working.
- Collaborate with a team of healthcare professionals to assess and treat patient conditions.
- Develop individualized treatment plans that articulate clinical reasoning, including assessment, treatment, and goals.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and family members on coping strategies and techniques for maintaining independence.
- Maintain accurate patient records and document the results of treatments.
- Ensure compliance with all regulatory and organizational standards.
- Work independently and as part of a team to deliver high-quality care.
