Position Summary:
Evaluate, design, and implement a patients plan of care; and participagte in local community service.
Each Plan of Care is created to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.
Repsonsbilities: The below is not an exhaustive list of duties and you will be expected to perform different tasks as necessitated by your changing role within the organization and the overall business objectives of the organization. Manage and monitor full patient cycle: perform an initial evaluation, establish a treatment plan, progress treatment and goals, communicate to physician, discharge, and follow-up.
Accompany patients during their physician visits Mentor new clinic staff Complete patient documentation in a timely manner including but not limited to, initial evaluations, progress notes, discharge summaries, daily S.O.A.P.
notes, functional status reports, functional capacity evaluations and insurance/plan of care forms as necessary Provide local community service such as hosting a booth, presenting in-services, provide coverage at special events, perform complimentary screenings, etc Establish and develop professional relationships with Athleticos referral sources including, but not limited to, physicians, PAs, ARNPs, chiros, case managers, etc.
Qualifications: Education: Degreed from an accredited Physical Therapy Program
Work Experience: 1-2 years of Athletico PT experience
Certification/Licensure: Must have an active clinician license or compact privileges in the state where services will be provided prior to beginning assignments; no temporary state licenses will be accepted
Knowledge and Technical Skills: Current CPR Certification Language Skills: Ability to read, write and speak English proficiently HR Use Only: The physical demands and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
