Physical Therapist Job Responsibilities: Develop and implement the Physical Therapy plan of care for facility residents and short-term patients. Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care. Assist as needed with patient screenings for the need for Physical Therapy. Communicate progress and any barriers toward PT goals with interdisciplinary team members. Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements. Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods. Provide and document patient, family, and caregiver training regarding each PT patient s specific plan of care and recommendations for follow up.
