**Job Title:** Physical Therapist - Home Health

**Job Specialty:** Physical Therapy Home Health

**Job Duration:** 13 weeks, with an option for a 26-week contract

**Shift:** Day 5x8-Hour (8:00 AM - 4:30 PM)

**Guaranteed Hours:** Facility can call off up to one shift per pay period if needed

**Experience:** 1 year of skilled home health experience required

**License:** Pending license accepted

**Certifications:** BCLS

**Must-Have:**
- Primary series COVID vaccination plus one booster
- Oasis proficiency

**Job Description:**

- Provide skilled physical therapy services in home health settings.
- Manage a caseload with a 1:1 ratio, visiting up to 6 patients per day.
- Utilize EPIC for charting and documentation.
- Drive to client's homes; occasional maximum radius of over 50 miles.
- No weekend or on-call duties required.
- Must have personal transportation.
- Facility groups client visits to minimize drive time.
- Float between facilities within the state as needed.
- Wear any color scrubs as attire.
- Open to first-time travelers.
- Local candidates are welcome and considered within a 25-50 mile radius at the same rate.
