**Job Title:** Physical Therapist - Home Health

**Job Duration:** 13 weeks (Open to 26-week contract)

**Shift:** Day 5x8-Hour (08:00 - 16:30)

**Guaranteed Hours:** Per contract, the facility can call off one shift per pay period if needed

**Experience:** 1 year of skilled home health experience required

**License:** Pending license accepted

**Certifications:** BCLS

**Must-Have:** Primary series COVID vaccination plus one booster

**Job Description:**

- Provide skilled home health physical therapy services in Anchorage and surrounding areas.
- Work independently with a patient ratio of 1:1, visiting up to 6 patients per day.
- Utilize EPIC for charting and documentation.
- Required to comply with primary COVID vaccination guidelines.
- Maintain your own transportation for patient visits.
- Typically operate within a maximum radius of 50+ miles, with efforts to minimize drive time.
- Experience and proficiency with OASIS is required.
- Wear any color scrubs as professional attire.
- No weekend work or on-call duties required.
- Acceptance of local candidates within a 25-50 mile radius at the same rate.
- Facility cannot accommodate block schedules, 4x10s, or 3x12s.
- Traveler expected to have reliable transportation.
- Completion of 40 hours of non-billable orientation required.
- Opportunity to float between all facilities in the state.
