**Shift:** Day 5x8-Hour (08:00 - 17:00), Monday to Friday

**Guaranteed Hours:** No guaranteed hours

**Experience:** Minimum 1 full year of Adult Home Health experience

**License:** Must have a valid license prior to starting

**Certifications:** CPR, BLS

**Must-Have:**
- Proficiency in HomeCare HomeBase (HCHB) used within the last 6 months
- Reliable transportation, a current driver's license, and agency-required liability insurance
- Understanding that there are no guaranteed hours with this contract
- Must be willing to travel up to 75 miles from the home clinic
- 30 points per week productivity expectation

**Job Description:**
- Provide, coordinate, and direct home nursing care according to physicians' orders
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Responsible for home visits and periodic office meetings
- Assignments include Start of Care, Wound Care, Follow-up Visits, etc.
- Drive to patients' homes daily and to the clinic for weekly meetings
- Complete all documentation within 24 hours of the patient appointment
- Float to nearby clinics to meet financial needs if patient census is low
- Expected to accept every patient on the schedule regardless of travel distance between appointments
- Complete a retraining course if previously used HCHB outside of the last 6 months
- Must be open to on-call rotations

This role requires flexibility, a willingness to travel extensively within the designated territory, and a commitment to maintaining timely and accurate documentation of patient care.
