**Job Description:**
- Accurately assess, diagnose, and identify deficits in a patient's ability to move and perform functional activities in daily life.
- Develop individualized treatment plans with patient-centered goals to reduce pain, restore function, and prevent disability.
- Work with individuals to prevent the loss of mobility, serving outpatient, acute, and swing bed patients.
- Perform patient evaluations through obtaining history, conducting relevant systems reviews, and administering specific tests and measures.
- Establish individualized plans of care and select treatment techniques according to physician orders.
- Conduct ongoing reassessment of the patient's response to treatment.
- Initiate and manage discharge planning from the first visit throughout treatment.
- Supervise and delegate tasks to ancillary staff and students in line with state laws and department policies.
- Complete legible, accurate, and clear documentation within established timeframes and per accreditation and insurance regulations.
- Communicate with patients, families, caregivers, and healthcare teams to maximize the benefit of care.
- Assess educational needs and provide education to patients, families, and caregivers.
- Engage in professional growth and development through educational programs and current literature.
- Participate in quality improvement activities.
- Provide a safe environment for patients, visitors, and staff, complying with approved safety standards.
- Function within the scope of practice as defined by the state practice act.
