**Experience:** Minimum of 1 full year Adult Home Health Experience or 1 full year Hospice experience required

**License:** Must have a valid license in hand

**Certifications:** CPR, BLS required

**Must-Have:**
- Proficiency in HomeCare HomeBase EMR, used within the last 6 months
- Proficiency in OASIS for RNs, PTs, and OTs
- Reliable transportation with a valid driver's license and agency-required liability insurance
- Willingness to travel up to 75 miles from the home clinic

**Job Description:**

- Provide, coordinate, and direct the provision of home nursing care according to physician orders and agency policies and procedures through the competent application of the nursing process.
- Responsible to the Clinical Manager, directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Expected to handle an average workload of 5-10 patients per day, equating to roughly 6-8 points.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Territory coverage includes areas from Bangor Northwest to Dover, and Bangor Northeast to Lincoln.
- Dress Code: Scrubs required.
- Maintain proficiency in EMR systems used for documentation and charting.
- Open to a weekend rotation approximately once a month; no on-call requirement.
- Must be open for on-call rotations with prior discussion during the interview.
- Local candidates are accepted at the same rate.
