**Job Description:**

- Provide skilled physical therapy services in a home health setting.
- Develop individualized treatment plans for patients based on assessments.
- Supervise and assess the progress of patients' rehabilitation efforts.
- Coordinate with a multidisciplinary team to ensure comprehensive patient care.
- Educate patients and families on best practices and exercises for continued health.
- Maintain accurate patient records and document treatment progress.
- Adapt therapy plans to suit individual patient needs and changes in condition.
- Ensure compliance with all relevant health and safety regulations.
- Demonstrate excellent communication and organizational skills.
- Contribute to the development and improvement of the rehabilitation program.
