**Certifications:** CPR, BLS

**Must-Have:**
- Proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months
- Experience with Oasis for RNs, PTs, and OTs

**Job Description:**
- Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Travel for home visits and periodic office meetings; must have reliable transportation, a current driver's license, and agency-required liability insurance.
- Average number of patients per day: 5-8.
- Assigned between 5-10 patients per day, equating to roughly 6-8 points.
- Provide different types of care, including Start of Care, Wound Care, and Follow-up Visits.
- Must be willing to travel up to 75 miles from the home clinic; mileage reimbursed at the IRS Standard rate.
- No weekend or on-call requirements in most locations; discuss specifics during the interview.
- Experience working with PointCare EMR systems.
- Cover territory surrounding the specified care center to assist with workload due to a lack of full-time PT.
- Locals accepted at the same rate.
