**Job Title:** Skilled Nursing Physical Therapist

**Guaranteed Hours:** 40 hours per week

**License:** Must be licensed as a Physical Therapist in the states of practice
**Certifications:** Department-specific competencies and certifications as required
**Must-Have:**
- Ability to supervise 1 Physical Therapist Assistant
- Familiarity with Netsmart for Home Health and Net Health for SNF, training available
- Valid driver's license as applicable
**Job Description:**
- Plan, organize, and deliver physical therapy programs to help clients/patients recover mobility, relieve pain, increase strength, and prevent deformities.
- Conduct examinations, evaluations, and interventions for patients affected by injury or disease.
- Utilize tools and techniques to administer physical therapy interventions, ensuring safety safeguards.
- Document patient information for evaluation and establish therapeutic plans, modifying as necessary.
- Educate clients/patients and their family members about treatment plans, schedules, and self-exercises for home continuation.
- Utilize appropriate age-related treatments and evaluations for diverse patient needs.
- Incorporate current research into the daily treatment of clients/patients.
- Focus on interventions in post-acute, long-term care, and home health care environments.
- Demonstrate proper documentation in accordance with departmental guidelines and regulatory requirements.
- Exhibit flexibility with varying work schedules and assignments.
- Participate in activities promoting the department and facility, interacting with various stakeholders.
- Supervise assistants, aides, and students in providing patient care and services.
- Provide ongoing education and training to peers in areas of expertise.
- Establish and maintain productive working relationships within and outside the department.
- Identify and mitigate common patient safety risks.
- Analyze and record therapeutic progress, recommending necessary adjustments.
- Adhere to legal regulations and organizational policies regarding patient health information.
This opportunity includes paid drive time and mileage between facilities and during home health travel with the expectation to cover multiple care settings. The schedule allows for flexibility, including the option to work Sunday through Thursday to have Friday off.
