**Experience:** Minimum of One Year of Recent Hands-On Experience as a Physical Therapist

**License:** New York State PT License

**Certifications:** Basic Life Support Certification

**Must-Have:** Valid State Driver's License and Reliable Transportation

**Job Description:**

- Review patient charts and medical history
- Develop a personalized treatment plan
- Teach therapeutic exercises and stretches to improve patient conditions
- Use hands-on therapy to promote healing
- Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
- Work closely with a supportive patient care team, including physicians, nursing, social services, pastoral care, home health aides, and others for assigned caseload
- Provide patient and family education for health promotion as directed by physician's plan of treatment
- Provide care to the utmost standard

- Team covers Southern Suffolk; Bayport, Bellport, Blue Point, Brookhaven, Farmingville, Holbrook, Holtsville, Lake Ronkonkoma, Patchogue, Ronkonkoma, and surrounding areas.
