**Certifications:** CPR, BLS

**Must-Have:**
- Proficiency in HomeCare HomeBase EMR within the last 6 months
- Experience with OASIS for RNs, PTs, and OTs within the last 6 months
- Ability to cover territory within a 75-mile radius
- Vehicle, valid driver's license, and auto insurance

**Job Description:**
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies through the competent application of the nursing process
- Responsible to the Clinical Manager
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct home visits and attend periodic office meetings
- See an average of 5-8 patients per day, with productivity expectations of 30 points per week
- Handle types of cases such as Start of Care, Wound Care, and Follow-up Visits
- Complete all documentation within 24 hours of patient appointments
- Be open to floating to nearby clinics if needed due to low patient census
- No on-call requirement
- Be willing to travel up to 75 miles from the home clinic
- Local candidates accepted at the same rate
- Accountable for accepting every scheduled patient regardless of the previous appointment's location.
