Job Title: Physical Therapist - Home Health

Guaranteed Hours: 40 hours/week

Experience: 1 year required

License: Pending state license accepted

Certifications: Basic Life Support (BLS)

Must-Have:
- Must work every other weekend
- Knowledge of EPIC charting system

Job Description:
- Provide home health physical therapy services to geriatric and middle-aged patients; no pediatric patients
- Manage a daily caseload of 5-6 units, ensuring quality care for patients with orthopedic, cardiac, neuro, pulmonary, and diabetes diagnoses
- Exhibit case management skills, independence, and superior time management and organizational skills
- Collaborate effectively with a multidisciplinary team and supervise physical therapist assistants
- Set patient-centered goals and maintain professionalism and excellent communication skills
- Demonstrate flexibility, a positive attitude, and the ability to deliver high-quality care
- Engage with the orientation program with a preceptor

Additional Requirements:
- Fit testing required; travelers must have suitable facial hair for testing or be expected to shave
- No medical marijuana cards accepted
- Background checks required; certain background issues may not be accepted
- Flu vaccine required with exemptions only for medical or religious reasons
- Willingness to work 2 out of the 3 major holidays: Thanksgiving, Christmas, and New Year's Day
- Will not float within the facility or to sister facilities
- Not open to accommodating a block schedule
- Return staff must wait 1 year before returning as travelers
- Must indicate interest in longer travel contracts if applicable
