**Job Description:**
- Coordinate and direct patient care services for patients of all ages to ensure that their needs are met.
- Perform patient evaluations and assessments, establish and implement plans of care, and modify treatment plans to ensure patients achieve their goals.
- Educate patients and caregivers to optimize patient outcomes.
- Produce timely and quality documentation to support the provision of care, aligned with professional association and regulatory guidelines.
- Collect and analyze data from selected outcome measures to monitor patient progress and the effectiveness of interventions.
- Demonstrate an understanding of treatment costs and financial aspects related to quality and efficiency, including budgeting, staffing, billing/coding, and procedural planning.
- Participate in ongoing professional development to maintain a clinical practice level consistent with best practice standards.
- Contribute to departmental, hospital, and community activities that promote department operations and program development.
- Perform other duties as assigned to support the overall effectiveness of the department and the organization.
