**Job Description:**

- Provide physical therapy services to patients in their homes, focusing on critical needs and rehabilitation.
- Conduct comprehensive assessments of patients' physical abilities and limitations.
- Develop and implement individualized treatment plans to improve patient mobility and functionality.
- Collaborate with other healthcare professionals to ensure a coordinated approach to patient care.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families on exercises and lifestyle changes to aid recovery.
- Maintain detailed documentation of patient treatment and progress.
- Comply with healthcare regulations and maintain patient confidentiality.
