**Job Description:**

- Join an in-house therapy team focused on providing high-quality health care.
- Work with smaller daily caseloads to optimize time spent with patients.
- Handle high-acuity cases responsibly and receive program and certification support.
- Develop and implement physical therapy care plans for both long-term residents and short-term patients.
- Supervise Physical Therapy Assistants in delivering care plans as needed.
- Conduct patient screenings to assess the need for physical therapy.
- Communicate progress and any barriers toward therapy goals with the interdisciplinary team.
- Ensure timely completion of PT documentation in compliance with regulatory, licensing, and payor requirements.
- Maintain rehab gym equipment and treatment areas safely, employing good infection prevention methods.
- Provide and document training for patients, families, and caregivers regarding specific care plans and follow-up recommendations.
- Specialize in geriatric care within long-term care, nursing home, and rehabilitation center environments.
