**Job Description:**
- Effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care.
- Develop an appropriate plan of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practice to support clinical interventions.
- Administer various procedures as part of the rehabilitation plan, including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Assess progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness; modify services in relation to patient evaluation and make referrals based on outcomes.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Provide a comprehensive discharge summary of services per regulatory and clinical requirements.
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
- Accurately record treatment minutes for services provided per all regulatory and clinical requirements.
- Participate in required meetings, including but not limited to patient care conferences, utilization review meetings, and rehabilitation meetings.
- Participate in facility and company-required trainings, in-services, and conferences.
- Obtain any necessary DME or supplies per regulatory and clinical practice requirements.
- Adhere to APTA and State Required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
