**Experience:** Accepting new graduates or Clinical Fellowship Year (CFY) candidates. Skilled Nursing Facility (SNF) experience preferred.

**License:** State License Verification Required

**Certifications:** CPR Certification (fundamental facilities)

**Must-Have:**
- Completion of prescreen and attachments
- Resume and license verification
- Three references
- Negative drug screen results

**Job Description:**
- Fill out and attach the prescreen form upon application submission.
- Attach resume and license verification with the application; submissions missing these will be declined.
- Experience with Rehab Optima preferred.
- Applicants must complete a credentialing checklist including:
- County Criminal Check (7 years for all counties lived and worked)
- Federal Criminal Check (7 years or more)
- Multi-State Criminal Search (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray
- SAM Search
- CPR certification
- Position urgency is classified as Rapid Response Crisis.
