**Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

**License:** Must have license in hand

**Certifications:** CPR, BLS

**Must-Have:**
- Recent specialty experience
- Proficiency with HomeCare HomeBase (HCHB) and Oasis within the last 6 months
- Reliable transportation and a valid driver's license
- Willingness to travel up to 75 miles from the assigned care center

**Job Description:**
- Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Manage an average of 5-8 patients per day, with productivity expectations of 30 points per week.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Complete all documentation within 24 hours of patient appointments.
- Travel time between patient homes is compensated, but the commute to/from the first/last patient is not billable.
- Adhere to a mileage reimbursement policy at the IRS standard rate.
- Accept assignments in a range of territories, primarily in specific counties.
- Be open to floating to nearby clinics if the patient census is low.
- Attend to territory needs created by workload and growth demands.
