**Job Description:**

- Perform patient care assessment and treatment including initial evaluations and motor skills assessments.
- Develop and implement realistic rehabilitation plans of care.
- Review and document physician referrals, patient medical records, prognosis, treatment response, and progress.
- Prepare written materials such as evaluations, plans of care, and progress notes while maintaining patient confidentiality.
- Facilitate patient mobility using safe handling equipment and manage treatment equipment.
- Adapt environments to support patient mobility and address deficits.
- Evaluate treatment effects and adjust to maximize benefits, promoting social adjustment.
- Educate patients and families on at-home care procedures and health maintenance.
- Train caregivers and patients to address needs during and after therapy.
- Recommend environmental changes to support patient work or living contexts.
- Provide clinical education opportunities and direction for physical therapist assistants and techs.
- Collaborate with the rehabilitation team to coordinate programs and activities.
- Assist team members with workload, orient new employees, and mentor students and other personnel.
- Seek professional growth opportunities and participate in quality improvement activities.
- Maintain and develop clinical competencies in physical therapy techniques.
