**Job Description:**
- Conduct 5 home visits per 8-hour day, averaging 5 cases daily
- Utilize Epic Systems Corporation - EpicCare Inpatient - EMR for patient charting
- Participate in orientation every two weeks prior to start
- Work within a Level 3 trauma unit with a focus on post-acute care
- Assist in providing quality patient care and ensure treatment plans are effectively implemented
- Ability to find alternative housing solutions if relocating with pets
- Cancellation policy: 1 shift cancellation for 12 weeks or less assignments; 2 shifts for 13-18 week assignments; 3 shifts for any contract exceeding 19 weeks
