**License:** Current unencumbered Physical Therapy license in Oregon

**Certifications:** Healthcare Provider Level AHA BLS required

**Must-Have:** Ability to travel to multiple points of care

**Job Description:**
- Evaluate and treat patients with injuries or conditions affecting physical mobility or functional independence.
- Provide a variety of outpatient services, including sports and orthopedic rehab, pediatric rehab, neurological and stroke rehab, balance and dizziness rehab, wheelchair seating, and care for neck, back, and extremities.
- Participate in required holiday coverage.
- Utilize EMR system (EPIC) for patient records and documentation.
- Contribute to maintaining a supportive and collaborative environment for rehabilitation services.
