**Job Description:**
- Balance of 80% outpatient and 20% inpatient responsibilities
- Plan and implement specific treatment programs for patients according to the principles and practices of Physical Therapy
- Evaluate patients and establish a written plan of treatment with short and long-term goals
- Maintain appropriate documentation records, such as daily logs, progress notes, and discharge notes
- Ensure effective communication within and between departments, as well as with physicians
- Participate in maintaining a professional and therapeutic environment within the treatment setting
