Travel Physical Therapist (Pt) In Monroe, Ga

**Guaranteed Hours:** None

**Experience:** Minimum 1 full year of Adult Home Health or Hospice experience

**License:** Must have license in hand

**Certifications:** CPR, BLS

**Must-Have:**
- Proficient in HomeCare HomeBase (HCHB) and used it within the last year
- Proficient in OASIS for RNs, PTs, and OTs
- Reliable transportation with a current driver's license and agency-required liability insurance
- Willingness to travel up to 75 miles from the home clinic

**Job Description:**
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct home visits and attend periodic office meetings
- Manage an average of 5-8 patients per day, equivalent to approximately 6-8 points
- Handle various cases, including Start of Care, Wound Care, and Follow-up Visits
- Serve both patients' homes and care facilities
- Engage in on-call rotations as required
- Compensated for travel between patient homes at the IRS Standard rate
- Complete around 25-30 weekly units of productivity expectations
- Utilize HomeCare HomeBase, Oasis, and Outlook EMR systems

This is a contract assignment with no obligation for hire upon completion. Local candidates are accepted at the same rate. You must not be a current direct employee of any Amedisys location if applying as a traveler.
